Ball gloves are measured from the tip of the index finger down to the heel: about 11 to 11.5 inches for infielders, 12 to 12.75 for outfielders and pitchers, and larger for slo-pitch and fastpitch gloves. Catchers and first basemen use mitts built for the job.
Bats are sized by length and by weight drop. A bat that is too heavy slows the swing; most players are better served by one they can hold straight out to the side for ten seconds.
